This page is intended to provide a resource for folks who wish to start working with Kellie Butler as she moves into private practice.

Here are the steps to start working with Kellie:

1. Fill out the form on this page.

2. You’ll be emailed a link to the client portal.

3. Click on that link, go in and create a password for yourself.

4. Kellie will email you a link to the intake paperwork to fill out in the client portal.

5. Kellie will get in touch with you to set up a date for your first appointment.
